Addison County Home Health & Hospice
New Haven, Vermont

Addison County Home Health & Hospice, a nonprofit VNA founded in 1968, was the first hospice agency to partner with Dinners With Love. They provide home care services that include skilled nursing, rehabilitation services, and medical social work, as well as ancillary support services that promote healing, quality of life and independence.

In 2018, Dinners With Love volunteers delivered 618 meals to ACHHH’s hospice patients and their families.

BAYADA Hospice​
Rutland, Vermont

Since 1975, BAYADA Hospice has provided clinical care and support services at home for children and adults of all ages. BAYADA has 300 offices in 22 states, including several in Vermont.

In 2018, Dinners With Love volunteers delivered 524 meals to BAYADA Rutland's hospice patients and their families.

BAYADA Hospice
Colchester, Vermont

Since 1975, BAYADA Hospice has provided clinical care and support services at home for children and adults of all ages. BAYADA has 300 offices in 22 states, including several in Vermont.

In 2018, Dinners With Love volunteers delivered 84 meals to BAYADA Shelburne's hospice patients and their families.

Caledonia Home Health Care & Hospice
St. Johnsbury, Vermont

Caledonia Home Health Care & Hospice (CHHC) is Northern Counties Health Care’s Medicare and Medicaid certified comprehensive Home Care and Hospice division, proudly serving the residents of Caledonia and southern Essex counties, and the Town of Greensboro since 1968.

​Caledonia launched their Dinners With Love program in July 2019!

Orleans/Essex VNA & Hospice
Newport, Vermont

Serving the Northeast Kingdom of Vermont since 1969, the Orleans/Essex VNA & Hospice provides professional home health or hospice care in the comfort and privacy of patients' own homes. They work hard to preserve their patients' dignity and provide education to help improve their quality of life. They accomplish this by promoting wellness and comfort, preventing disease complications and re-hospitalizations, and providing appropriate high-quality services to meet individual needs.   

In 2018, Dinners With Love volunteers delivered 24 meals to OEVNA's hospice patients and their families. 

The University of Vermont Health Network Home Health & Hospice
Colchester, Vermont

It is the mission of UVMHN Home Health & Hospice to help people live their fullest lives by providing innovative, high-value, compassionate care wherever they call home. 

In 2018, Dinners With Love volunteers delivered 638 meals to UVM's hospice patients and their families. 

VNA & Hospice of the Southwest Region - Bennington
Bennington, Vermont

For more than 70 years, the Visiting Nurse Association & Hospice of the Southwest Region (VNAHSR)  has served vulnerable populations such as at-risk children, the elderly, those with disabling and chronic illness, and people facing the end of life. Founded in Rutland, Vermont, the VNAHSR is a non-profit, Medicare-certified home health and hospice agency delivering a wide-range of advanced medical care with compassion, dependability, and expertise to people of all ages. VNAHSR is one of Vermont’s largest not-for-profit community health provider of home health, hospice and community services. More than 350 certified home health aides, rehabilitation therapists, social works and visiting nurses bring their skilled care and compassion to people who need it most each day. Whether in their homes, or through one of our many community clinics, VNAHSR bridges gaps to secure the health care people most need.

In 2018, Dinners With Love volunteers delivered 574 meals to VNAHSR Bennington's hospice patients and their families. 

VNA & Hospice of the Southwest Region - Rutland
Rutland, Vermont
